Optimizing upper extremity rehabilitation in children with unilateral spastic cerebral palsy: adjunctive strategies and an integrative framework.

Abstract

Purpose Intensive, motor learning-based interventions such as constraint-induced movement therapy and bimanual training improve upper extremity outcomes in children with unilateral spastic cerebral palsy, yet required effective dosages are difficult to deliver in clinical settings. This paper conceptualizes how adjunctive strategies may optimize upper extremity rehabilitation by addressing translational constraints. Materials and methods This perspective integrates selected empirical findings and theory-driven interpretation informed by our experience in pediatric neurorehabilitation and motor learning. Rather than offering a comprehensive review, we use representative evidence to motivate a novel framework and provide pragmatic judgements for clinicians and researchers. Results Although high-dose, motor learning-based therapies remain the gold standard to improve upper extremity function, implementation is limited by time, cost, and service-delivery constraints. Adjunctive strategies may enhance feasibility through complementary mechanisms: dose modification (e.g., neuromodulation paired with training to enhance responsiveness to practice); and dose distribution (e.g., home-based training, telerehabilitation, and recreational physical activity, which redistribute structured motor practice across varied contexts). Together, these approaches may support sustained practice, motivation, and participation beyond the clinic. Conclusion We propose an integrative framework that positions adjunctive strategies as complements to, rather than replacements for, intensive motor learning-based interventions, supporting upper extremity improvements with fewer clinical hours.
